10-2pm COVID VACCINE EQUITY CLINIC at the Second Chance Center
- Register online and pick an appointment time that works best with your schedule. Before you arrive, review your appointment confirmation email for any additional information you may need to bring.
- When you get a vaccine, you and your vaccinator will both need to wear masks that cover your nose and mouth. Stay 6 feet away from others while inside and waiting in line.
- You will receive a paper copy of a fact sheet that tells you more about the vaccine you will receive.
- You will also receive instructions on how to register with the V-SAFE After Vaccination Health Checker where you will be able to report any side -effect symptoms to the CDC.
- After receiving a COVID-19 vaccine, you will be monitored on-site for at least 15 minutes to check for any reactions.
- You will receive a vaccination card that tells you what COVID-19 vaccine you received, the date you received it, where you received it, and the date to return in the event you receive a 2-dose vaccine. Keep your vaccination card in case you need it for future use, this will be your only proof of vaccination. Consider taking a picture of your vaccination card as a backup copy.
*Please remember to bring a valid form of identification and an insurance card, if applicable.
**Please note that you must register a separate appointment for each individual that wants to be vaccinated.
